Licensed Master Social Worker
Location: Pompton Plains, NJ

Pay: $2,560 to $2,660 per week
Shift Information: Days - 5 days x 8 hours

Assesses patients’ psychosocial needs and develops appropriate interventions and treatment goals.
Provides emotional and psychosocial support to patients and families during illness and life changes.
Participates in multidisciplinary rounds and advocates for patients and families during advance care planning.
Coordinates discharge planning and post-hospital care, including SNF, acute rehab, LTAC, dialysis, hospice, and housing resources.
Connects patients with community resources, government services, financial assistance, and transitional housing.
Documents psychosocial assessments, family interventions, and discharge plans in compliance with hospital, state, federal, and Joint Commission requirements.
Collaborates with healthcare teams and community organizations to ensure continuity of care.
Maintains patient confidentiality and contributes to service improvement initiatives.
Key skills: Excellent communication, negotiation, and conflict-resolution skills; knowledge of social work regulations, clinical practices, and healthcare technology.
Physical requirements: Must be able to perform duties involving standing, walking, handling, and lifting, with appropriate visual acuity and hearing.
Education: Graduate of an accredited school of Social Work
Experience: Previous medical hospital social work
